Essentially, we provide health insurance with pre-tax dollars and the opportunity to receive coverage for FREE when you earn the Wellness Rate. Enroll in the Smart Choice Plan and open an HSA to receive contributions from Trilogy. Trilogy matches 100% of employee contributions up to $300.00 per plan year (4/1 – 3/31) and makes weekly contributions. Your contributions are made through payroll deductions with pre-tax dollars. HSA’s have a triple tax advantage; (1) your contribution is withheld before taxes, (2) your account earns interest tax free and (3) when you withdraw the funds for health expenses, no tax is due.

Flexible Spending Accounts
Employees who elect the PPO plan may enroll in the Healthcare Flexible Spending Account (FSA), or the FSA may be elected as a stand-alone benefit if you are not enrolled in Trilogy’s health insurance. The money is withheld from your regular paycheck pre-tax and deposited into your account. The maximum annual contribution is $2,750.00 for 2020. With a Healthcare FSA, you can use tax-free dollars in your account for reimbursement of eligible medical, dental, and vision expenses for you and your dependents. Any unused funds are forfeited. Anthem LiveHealth Online
LiveHealth Online gives you access to video visits with a board-certified doctor, psychiatrist, or licensed therapist from your smartphone, tablet, or computer from home or anywhere!
All LiveHealth Online visits are FREE to employees and covered dependents enrolled in the Anthem medical plan.

Anthem utilizes calls to communicate important health information, appointment or healthcare reminders, or to let you know of a wellness program you may be eligible for. These calls are always confidential.

Tuition Reimbursement Program
Trilogy provides full-time employee’s tuition assistance during employee’s pursuit of education related to their job in order to expand their education, achieve higher levels of personal fulfillment, and increase job satisfaction.

MetLife CI & ACC $50.00 Health Screening Benefit
MetLife provides a $50.00 Health Screening Benefit (HSB) to enrollees who provide proof of an annual wellness screening. The HSB can be redeemed for employees, and their dependents, covered under MetLife’s Critical Illness or Accident Coverage Plans. The $50.00 is provided for each type of coverage, so if you are enrolled in both plans, you will receive two $50.00 checks, just for submitting proof of screening. Simply call and provide verbal verification of completion over the phone or fill out the attached HSB Form.
Accident Insurance (Off-The-Job)
Provides you with a lump-sum payment when you suffer a covered injury or undergo covered testing, medical services, or treatment, and meet the group policy and certificate requirements. Premiums are paid through payroll deduction. Payments are made directly to you to use as you see fit. They can be used to help pay for medical plan deductibles and copays, out-of-network treatments, for your family’s everyday living expenses, or whatever you need while recuperating from an accident.
Critical Illness Insurance
Helps cover the extra expenses associated with a serious illness. This coverage provides a lump-sum payment of $15,000 in Initial Benefits upon diagnosis. The Total benefit amount available is 3 times the Initial Benefit Amount, ($45,000.00) in the event you suffer more than one Covered Condition. Critical Illness insurance provides you with a lump-sum payment upon diagnosis of the following conditions:
- Full Benefit Cancer
- Alzheimer’s Disease
- Partial Benefit Cancer
- Kidney Failure
- Major Organ Transplant
- Heart Attack, etc.

The Trilogy 401(k) Retirement Savings Plan is administered by Fidelity Investments with Financial Advisement from Merrill Lynch Bank of America. You can save state and federal income taxes on your contributions to the 401(k) plan. The 401(k) plan has pre-tax and Roth contribution options. Your pre-tax contributions and earnings are tax-deferred until you take a distribution. You are eligible to contribute the first of the month following 60 days of employment and age 21. Trilogy will match $0.25 on every $1.00 an employee contributes up to 10% of their weekly pay*.

If you are going to be absent from work for more than 3 days you may be eligible for FMLA (Family and Medical Leave).
All active employees working 15+ hours per week are eligible to enroll in short-term and or long-term disability coverage. Pre-existing conditions apply.
Short-term disability is designed to provide you with continuing weekly income while you are out of work due to a non-work related illness or accident, including pregnancy. Benefits begin following a 14 calendar day waiting period from your first date of absence related to the disability. Short-term disability insurance provides a benefit amount of 60% of your weekly pre-disability earnings, up to a weekly maximum benefit of $750.00, for as long as you remain disabled up to 24 weeks.
Long-Term Disability pays a benefit of 60% of your monthly income up to a maximum of $10,000.00. Benefits begin on the 181st calendar day of absence and may last for five years for accident/sickness. Pre-existing condition clause applies.
Basic Life Insurance
Trilogy offers Basic Term Life Insurance and AD&D for full-time employees through MetLife at no cost.
Voluntary Life Insurance (Self)
Trilogy offers voluntary life insurance in multiples of one to five times your annual salary up to a maximum of $250,000.00 of coverage (if approved).
Voluntary Life Insurance (Spouse and Dependent Children)
If you purchase voluntary life insurance for yourself, you may also purchase voluntary life for your spouse and/or dependent children. Spouse voluntary life benefits can be purchased up to 50% of your voluntary term life benefit, to a maximum of $50,000.00 or a flat $10,000.00 for your dependent child(ren).
